Do I need to make an appointment?
Our office works by appointment only, so that you are never waiting to see the doctor.
What is the parking like?
There is on street parking as well as a paid garage. There is also a Ralph’s market one block away.
What paperwork should I bring to my first appointment?
Please bring all previous diagnostics that were done. X-Rays, MRI’s, Nerve conduction studies, bloodwork etc. To save time in the office, download our New Patient intake paperwork. Fill it out and bring it with you.
What are your payment options?
We accept credit cards, checks, cash and Care Credit for monthly payment options.
